Sick and Safe Leave Request Form

Pursuant to Maryland law, employees are entitled to earn sick and safe leave at the rate of 1 hour for every 30 hours that an employee works up to a maximum of 40 hours per year. The year commences on and ends on January 1st of every year. An employee accrues earned sick and safe leave at a rate of one hour for every 30 hours the employee works, however, an employee is not entitled to earn more than 40 hours of sick and safe leave in a year. Outdoor Therapeutics has two employee types eligible for Sick and Safe Leave: Full-Time Employees are understood to work 36 hours and therefore accumulate leave at a rate of 1.2 hours per week. Part-Time Employees are understood to work 20 hours and therefore accumulate 40 minutes of leave per week. Please check with your supervisor about the amount of leave accumluated in your leave pool prior to completing this request to ensure you have sufficient leave to cover your needs.
